JOB SUMMARY

The Population Health Coordinator identifies and assists patients with obtaining appropriate post-acute care services. Once identified, the coordinator coordinates with various stakeholders to ensure a safe discharge transition for the patients. Coordinators assist in arranging post hospital follow up appointments. Provides supportive role to the case management department. This may include but not limited to the gathering and transmission of data to ensure appropriate patient outcomes. Interaction with the various stakeholders involved with coordinating care of patients. Strong communication, administrative and customer service skills are essential. 


ESSENTIAL DUTIES


Assesses electronic platforms to promote quality measures that achieve positive patient outcomes. Utilizes electronic worklist to identify and reach out to patients, families, and/or members of the multidisciplinary team for post-acute needs.

 

Collects and updates demographic information, as appropriate and ensures that complete and accurate documentation is recorded in the computerized database for each contact.

 

Maintains a patient centric, customer-driven, professional attitude. Demonstrates awareness of own values and attitudes and how they may affect information given to a client.

 

Functions independently and demonstrates leadership in decision making, communication, and problem-solving techniques to resolve clients’ questions/concerns. Collaborates with other members of the case management team as needed.

 

Other responsibilities as assigned that assists the case management team in ensuring a smooth and safe transitions of care through the health continuum.

Must be able to type accurately and the ability to type 35 wpm is preferred.

Good interpersonal skills and ability to deal with all levels of staff within the organization and community under varied conditions.

 

Ability to operate a variety of general office equipment and use of a computer.

 

Successful completion of annual job specific competencies and skill verification tools required.

 

Must be knowledgeable of healthcare insurance rules.
