• | Coordinates and cares for patients in inpatient and outpatient CardioPulmonary Rehabilitation utilizing specialized clinical expertise and resources to facilitate a high level of participant satisfaction and quality clinical outcomes. Performs all aspects of care in an environment that optimizes patient and environmental safety in accordance with applicable laws, regulations and compliance efforts. | • | Assesses, monitors, and prescribes exercise for participants according to established department policies. Creates and maintains patient individualized treatment plans in accordance with guidelines. | • | Coordinates and provides education specific to pulmonary patient needs. | • | Monitors and accurately documents patient response and progression to therapy, maintaining confidentiality of information and communicates appropriately to physicians. | • | Provides accurate documentation and billing throughout program. | • | Demonstrates critical thinking skills and uses sound judgement in evaluating and responding to emergent situations. | • | Assists with monitoring of Cardiac Rehabilitation patients within the department. | • | Shares expertise and knowledge with staff, developing their clinical, technical and professional skills and participates in the instruction and validation of technical competency within the department. |
