Obtains history and physical from the patient for the purpose of detecting and treating injury/disease and collecting potential forensic evidence. Maintains chain of custody and coordinates care with law enforcement and Rockford's Sexual Counseling Center. Documents assessment, evidence collection, and intervention with careful attention to legal considerations. Provides crisis intervention and client education regarding STD and pregnancy concerns. Must be able to take call and respond to call with presence at hospital within 60 minutes. | | | | | | | | |

Demonstrates effective documentation and communication skills. | • | Promotes safety and infection control procedures. | • | Provides care and education incorporating age-specific, cultural and spiritual needs of the patient and significant other into the plan of care. | • | Performs all aspects of care in an environment that optimizes patient and environmental safety and in accordance with applicable laws, and compliance efforts. |
