JOB SUMMARY

Plans, evaluates and administers Occupational Therapy services and care to clients with disorders in functional performance in their home as prescribed by the referring physician on an intermittent basis according to the Agency policies. Participates in continuous quality improvement activities and educational experiences in support of departmental philosophy and objectives, as well as, Health System initiatives.  Coordinates family involvement in assessment and treatment of the client.


ESSENTIAL DUTIES


Evaluates, reevaluates and assesses client’s functional performance as prescribed by the physician and establishes an individualized occupational therapy plan of care that integrates goals, treatment and outcomes in accordance with Agency guidelines.

 

In collaboration with the health care team, administers and develops client’s plan of care per physician’s orders incorporating age-specific, cultural and spiritual needs of the client and significant other into the plan of care.

 

Continually observes and reevaluates plan of care and treatment effects consulting with physician and other health care professionals, altering program as needed in order to provide quality client care.

 

Supervises assistants, students and homehealth aide in providing support functions according to Agency guidelines.

 

Reevaluates the client at regular intervals and makes adjustments to plan as necessary in collaboration with the treatment team. Participates in discharge planning and client care conferences as necessary.

 

Coordinates family involvement in the continuing care of the client and aids family in coping with traumatic illness. Educates patients, family and other health care providers on appropriate care modifying activities considering learner’s needs and capabilities.

 

Schedules client services and accurately documents client information and treatment following Agency policies and maintaining confidentiality of position acquired information.

 

Performs all aspects of care in an environment that optimizes client and environmental safety and in accordance with applicable laws, regulations and compliance efforts.

 


As needed, additional responsibilities reasonably within the scope of duties and physical requirements may be assigned.  ALL DUTIES AND REQUIREMENTS MUST BE PERFORMED CONSISTENT WITH THE UW HEALTH PERFORMANCE STANDARDS AND RESPECT FOR PEOPLE COMMITTMENTS.

An applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment.  An applicant who fails to provide this documentation will not be hired.  A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided.

Knowledge, Skills, and Abilities

Successful completion of annual clinical, age and job specific competencies and skill verification tools required.

 

Familiarity with relative value and limitation of equipment.

 

Must be skilled in a broad range of modalities with clear understanding of indications and contradictions for each.

 

Ability to effectively communicate orally and in writing in a caring, considerate and concerned manner.

 

Leadership skills.

 

Knowledge of IDPH, CMS and JCAHO rules and regulations.
