JOB INFORMATION | Effective Date: | 7/23/2020 | Entity: | Illinois | Job Code: | NI-2178HH | Job Title: | NI-Liaison, Home Health | FLSA Exemption Status: | Exempt | JOB SUMMARY | | | Responsible for educating physicians, acute care facility staff and other referral sources regarding the Home Health Care role in accordance with Agency policy. Functions as agency liaison for the referral process. Participate in continuous quality improvement activities and educational experiences in support of Agency philosophy and objectives, as well as, Health System initiatives. | | | | | | | | |
• | Coordinates intake from UW Health health System referrals and nursing homes for all home care services following established agency policies. Consults with and gathers information from members of the multidisciplinary team, client, family and referral sources to coordinate start of services and establish an initial care plan prior to home care admission. | • | Receives information from all referrals generated from the UW Health Health System or other sources. Enters client information and orders into computer to register clients and verifies insurance for new referrals. | • | Functions as a resource and customer service representative for hospital staff, clients, physicians and community on home care services. Triages all calls and provides appropriate follow-up during office hours. | • | Coordinates the client’s family in the assessment process ensuring that the needs for daily living are provided in the physical and social environment. | • | Meets with families and clients prior to discharge to explain the Agency’s policies and provide information on Medicare coverage, private insurance reimbursement or out of pocket expenses. |
