Participates in continuous quality improvement activities and educational experiences in support of departmental and divisional nursing philosophy and objectives, as well as, Health System initiatives. | | | | | | | | |
• | Triage incoming Health Connect phone calls from external customers and medical providers utilizing computerized database to provide appropriate clinical information and to facilitate referrals to primary care providers, specialists, healthcare facilities and community resources. | • | Utilizes clinical judgement, to provide sound nursing assessments for Health Connect callers following established guidelines and utilizing physician referral, class registration and nurse triage software. | • | Within 24 hours of initial call, initiates follow-up phone calls to appropriate Health Connect callers to assess and reinforce compliance. | • | Accurately documents information on all inquiries ensuring accurate legal and statistical tracking and reporting to other departments and maintains confidentiality of position acquired information. | • | Assists with the implementation and external marketing of approved consumer programs. | • | Maintains a current working knowledge of community managed care programs, physicians and group practices, SAHS programs and services and community medical resources in order to recommend appropriate referrals. Functions as a liaison between physician offices and consumers. |
