JOB SUMMARY

Provides individual and group therapy throughout the continuum of psychiatric services.   Develops, implements and documents the treatment planning process and contributes to a smooth transition through the continuum of care.


ESSENTIAL DUTIES


In collaboration with the multidisciplinary team, develops, implements and evaluates patient’s treatment plan and progress. Revises plan as needed involving patients, families, significant other and other service providers.

 

Completes initial case opening and contacts family as appropriate.

 

Facilitates and provides psychotherapy, psychoeducational and/or goals groups on a routine basis.

 

Accurately documents appropriate patient information and communicates on going with interdisciplinary team and referral sources.

 

Markets program and service to the community and participates in community activities.

 

Develops specific discharge plans and interventions (beginning at the time of admission intake) based on assessment of patient needs. Provides appropriate referrals to aftercare providers, providing the patient with all necessary information to complete the referral.

 

Performs all aspects of care in an environment that optimizes patient and environmental safety in accordance with applicable laws, regulations and compliance efforts.

 


As needed, additional responsibilities reasonably within the scope of duties and physical requirements may be assigned.  ALL DUTIES AND REQUIREMENTS MUST BE PERFORMED CONSISTENT WITH THE UW HEALTH PERFORMANCE STANDARDS AND RESPECT FOR PEOPLE COMMITTMENTS.
