Ensures linkage with other community agencies. Completes admission procedures for all programs within the Center for Mental Health (CFMH.) Performs telephone triage functions. Participates in continuous quality improvement activities and educational experiences in support of departmental and divisional nursing philosophy and objectives, as well as Health System initiatives. | | | | | | | | |
• | Conducts comprehensive psychosocial assessment of patient and significant other(s) as indicated discussing findings with on-call psychiatrist to determine appropriate level of care and/or disposition. | • | Provides appropriate referrals to community resources and monitors client compliance with agency recommendations. | • | Completes requirements to pre-certify participation in CFMH programs per established guidelines. | • | Performs all aspects of care in an environment that optimizes patient and environmental safety in accordance with applicable laws, regulations and compliance efforts. |
