JOB SUMMARY

The Clinical Program Coordinator (CPC) is a registered nurse who is accountable for working with physicians and staff to deliver safe, timely, effective, efficient, patient-centered, and equitable care to patients and families.  The CPC applies knowledge and skills by:

 

Providing direct patient care;

* Assessing competency of nursing medical assistant staff and technical staff;

*  Ensuring compliance with UWH policies and procedures as well as Nursing standards of practice;

*  Leading patient care programs in the areas of patient education, and performance improvement activities;

*  Identify, assist to develop and assist to implement operational improvement initiatives

*  Triaging patients and identifying resource availability;

*  Coordinating, implementing and conducting patient screening for risk factors prior to procedures;

*  Communicating effectively with physicians and other providers;

*  Assisting with change management, using tact, discretion and diplomacy;

*  Working with staff to facilitate problem solving, team work, and effective day to day clinic operations

*  Providing feedback to staff, faculty, and management.

 


ESSENTIAL DUTIES


Integrates the principles and philosophy of the professional nursing care delivery system into practice.

 

Serves as a role model to other nursing personnel by demonstrating effective direct patient care, leadership and professional behavior.

 

Reviews appointment schedule on daily and weekly basis and makes adjustments in support and staff schedules as needed.

 

Provides direct nursing care and patient education to patients/families, assuring continuity of care.

 

Serves as a liaison with the medical staff to identify issues of mutual concern, solve problems, review staffing plans and optimize clinic operations.

 

Participates in the recruitment process for the nursing, medical assistant and technical staff.

 

Enforces adherence to hospital and department policy and procedures.

 

Assist with staff scheduling on a regular basis.

 

Promotes open and effective communication among staff, medical faculty, other departments.

 

Is responsible for a safe and therapeutic environment for patients, visitors and staff.

 

Develops and utilizes a process for equipment utilization and supply management at the clinic.

 

Participates in budget development and monitoring for clinic.

 

Develops and implements an ongoing process for problem-solving that includes input from department faculty and appropriate hospital departments.

 

Develops and implements action plans to enhance patient satisfaction and access as well as provider productivity.

 

Acts as a consultant and clinical resource to clinic staff through provision of patient care and coordination of care.

 

Coordinates medical and nursing management of patient care in the clinic on a daily basis.

 

Supports patient advocacy and the patient’s Bill of Rights.

 

Develops, maintains and evaluates patient care standards for the clinic.

 

Monitors all nursing activities related to patient care.

 

Promotes a unit-based, multi-disciplinary quality improvement program that incorporates patient care standards and meets all regulatory guidelines (i.e. TJC, CMS, BQA, etc.) for ambulatory care.

 

As directed, participate in department level work groups and committees to address operational. quality and safety initiatives.

 

Develop, maintain and evaluate clinical procedures for the clinic

 

Creates an environment in which learning is supported and valued.

 

Interprets and supports the philosophy, objectives, and policies and procedures that guide personnel, patients and the public.

 

Participates in patient, family and staff teaching.

 

Meets personnel continuing education and developmental needs.

 

Serves as a clinical instructor in planning, guiding and evaluating medical and or nursing students’ clinical experience.

 

Develops appropriate written patient education materials.

 

Serves as a resource to staff in other clinics as appropriate.

 

Supports and/or participates in research programs approved by the Hospital and the University.

 

Supports and/or participates in community education and outreach programs.

 

Supports and/or participates in clinical research activities approved by the hospital.
