JOB SUMMARY

The Care Team Leader is an advanced level registered nurse who works collaboratively to plan, direct, and evaluate the care provided to patients. The Care Team Leader possesses expert clinical knowledge, which he/she applies to systematically and continuously collect and assess data in collaboration with the multidisciplinary team to provide therapeutic care for the patient. The Care Team Leader performs direct and indirect patient care to meet the clinical, educational, and coordination needs of the surgical patient population. The practitioner is active in establishing effective collaborative relationships across settings and with other disciplines. The Care Team Leader considers factors related to safety, effectiveness, resources, and cost in planning and delivering care and managing the work of the team.

 

The Care Team Leader will work closely with leadership and other care team members to establish goals and strategies for meeting the admissions and discharge needs of the patient, family, and other care providers. The Care Team Leader actively facilitates the work of the team and ensures delegation and supervisor of team member care in accordance with nurse practice acts and the professional nursing role. He/she will develop systems and processes to ensure optimal and ongoing efficiency.

 

The Care Team Leader utilizes research findings in his/her practice. He/she demonstrates leadership in improving organizational performance and actively works to apply clinical practice improvements. The Care Team Leader maintains expert clinical knowledge in nursing practice and assumes responsibility for the professional development of self, other nursing staff, and students.


ESSENTIAL DUTIES


Establishes an in depth database by assessing the behavioral and physical status of a variety of perioperative patient populations including interview, observation, and physical examination and seeks information that completes the database from multiple internal and external sources.

 

Implements safe, competent, and efficient patient care within policies, procedures, standards, and interventions as noted on the multidisciplinary plan of care. Coordinates completion of necessary components (i.e. consent) prior to the patient entering the operating room.

 

Initiates action to reduce or correct risk in response to data indicating urgent and/or emergent risk to the patient.

 

Facilitates flow in and out of the clinic.

 

Documents the nursing process to reflect a comprehensive and integrated approach to patient care.

 

Evaluates the patient’s responses to care based on the effectiveness of nursing interventions and in relationship to established outcomes and consults with medical staff.

 

Identifies learning needs of the patient and family and adapts standard information to provide individualized and comprehensive teaching.

 

The Care Team Leader collaborates with the patient, significant others, and health care providers in providing patient care. He/she delegates and supervises care in accordance with nurse practice acts and the professional nursing code.

 

Initiates multidisciplinary collaboration to positively impact the outcome of health provided.

 

Initiates steps to support and enhance the patient’s responsibility and self-determination in decision making concerning health, treatment, and wellbeing.

 

Involves the individual patient and family in identification of requirements/perceptions of learning needs and preparation to assume self-care on discharge.

 

The Care Team Leader considers factors related to safety, effectiveness, and cost in planning and delivering care. He/she demonstrates knowledge of the UW Health care delivery system as multidisciplinary and multifaceted with a variety of entry and access points along the care continuum.

 

Contributes to and supports the effective management of the unit.

 

Facilitates cost controls in the use of material and human resources.

 

Effectively utilizes technology to support practice and efficiencies in care delivery.

 

Identifies opportunities and participates in evaluations designed to measure the impact of new products and technology on patient outcomes or enhancements to the care delivery process.

 

The Care Team Leader acquires and maintains current knowledge in nursing practice and assumes responsibility of the professional development of self, other nursing staff, and students.

 

Contributes to the learning experience of students in cooperation with the instructor and other staff.

 

Works directly with preceptors to ensure new employee orientation is accomplished.

 

Assists in the orientation, instruction, direction, and evaluation of students assigned to the clinic.

 

Reviews staff documentation to ensure all requirements are met.

 

The Care Team Leader uses research findings in practice.

 

Participates in organizational improvement activities.

 

Identifies recurring clinical practice issues and contributes to the development of specific plans to address identified issues.

 

Incorporates changes in practice to reflect new advances and ensures effectiveness of the interventions.
