JOB SUMMARY

The RN Cardiovascular Device is a registered nurse who is independently accountable for the effective management and coordination of care of patients with Cardiac Implantable Electronic Device (CIED), pacemakers, ICD, and Implantable Loop Recorders. The RN will partner with the Electrophysiology (EP) team, Interventional Cardiologists, heart failure team, cardiothoracic surgical team, and other clinical staff throughout UW Health to coordinate the care of cardiovascular patients with CIEDs. The RN will coordinate care in both an inpatient and outpatient setting in addition to external local and regional healthcare facilities. The RN will provide and improve quality patient outcomes for the patient and families for the Heart Clinic and Device Clinic in collaboration with the multidisciplinary team. 

 

The RN Cardiovascular Device has a strong knowledge base of cardiovascular care and advanced heart diseases. The Coordinator will act with and as a physician liaison to local and regional referring physician providers and clinical staff. They will evaluate current clinical practices of referring providers and staff and provide education to ensure research-based best clinical practice is delivered to patients. The RN Cardiovascular Device provides leadership as a member of the multidisciplinary team, collaborating in the development, execution, and evaluation of the programs.

 


ESSENTIAL DUTIES


Conduct interviews with patient, family, and other health care agencies to obtain past medical records and social and medical history to assess their understanding of the plan of care regarding cardiac monitoring devices.

 

Independently perform CIED interrogation in the outpatient setting, including UW Health Madison Heart Clinics and UW Health Regional Specialtiy Clinics.

 

Independently perform CIED interrogation and device/patient monitoring in the inpatient setting including MRI, Radiation Therapy, ER, OR, and inpatient units.

 

Problem solve device function, settings, arrhythmia episodes, and interventions. Work closely with EP team and other multidisciplinary teams, ensuring quality CIED and medical follow up care.

 

Analyze clinical data and compose written CIED management summaries, clinic visit notes, and letters.

 

Communicate and problem solve with multidisciplinary team to solve complex patient care issues and CIED function.

 

Educate patient and family regarding all aspects of disease management and diagnostic and device management follow up.

 

Proactively review patient schedules to ensure efficiency in clinical workflows and identify and resolve potential conflicts.

 

Ensure appropriate resources are available to support patient care and procedures in both the inpatient and outpatient setting.

 

Educate patient and family on signs and symptoms that require a follow up telephone call or triage RN or MD Clinic visit.

 

Collaborate with medical faculty, nursing staff, and other health care agencies to facilitate patient and family needs.

 

Assist with accurate data collection and analysis to support Device Clinic charting platform, national registries, process improvement, and quality initiatives.

 

Serve as a resource to staff within UW Health, regional referring providers and clinical staff, the cardiovascular and electrophysiology laboratories, and inpatient and outpatient units.

 

Participate in planning for seminars, conferences, symposiums, or any educational event related to cardiovascular programs.

 

Obtain advanced nursing certifications and specialty education to advance the care of the cardiovascular patient based on evidence-based practice.
