JOB SUMMARY

The Social Work Supervisor provides leadership and supervision for the Social Worker group within the department. The Social Work Supervisor is responsible for the supervision and development of procedures and care provided by the Social Work staff, and for the professional oversight of the discipline. This includes monitoring the competency of staff members and the appropriateness of care provided to patients and their families. This position provides direct supervision of the Social Work practice, assuring service excellence and compliance with all applicable hospital and departmental polices, laws, and department specific regulations.   

 

The Supervisor assists with the hiring and development of new employees, as well as assessment of productivity and appropriateness of team resources.  This position is also responsible for the implementation and evaluation of performance improvement projects and related data, clinical procedures, protocols, research, and the education of the multi-disciplinary team members regarding social work procedures.  The supervisor is responsible for duties including education and mentoring of staff, managing social work clinic coverage and staff schedules.  The supervisor may also provide direct patient care as required by programmatic needs.

 

The  Supervisor works in collaboration with a wide variety of internal and external constituents, including (but not limited to) team members, physicians, and staff throughout the UW Health continuum of care,  patients, families, community services, and other health care systems.  S/he serves as an active member on relevant program, department, and other committees across the care continuum.

 


ESSENTIAL DUTIES


Oversees and prioritizes the day-to-day staff schedules/work shift assignments for the Social Workers, including patient care needs, coordinating paid-time off and approving biweekly timecards. This position is responsible for developing and maintaining the SW staff schedule.

 

Oversees and facilitates the recruitment, orientation, and training of SWs, assuring competency and facilitates relevant educational opportunities.

 

Assures that new SWs receive an orientation of sufficient duration and content.

 

Assures that SWs participate in relevant in-service education programs.

 

Completes regular chart reviews and performance appraisals for all direct reports and provides information to the Clinical Director regarding the performance of their team members.

 

Facilitates staff participation in workflow process improvements.

 

Manages technical, environmental, and financial resources effectively; provides input to leadership regarding budget process as needed by the program

 

Assists leadership in the development of standard operating procedures to guide the work of the SW team.

 

Provides information regarding supplies and staffing needs for consideration in the departmental budget.

 

Builds and maintains relationships with other departments across the UW Health continuum of care and partnering agencies in the community.

 

Reviews departmental objectives and assists in the development of long- and short-range goals in conjunction with the departmental leadership group.

 

Actively involved in performance improvement activities, including collaboration with data and IT resources on the development and use of data reports to monitor and improve services

 

Proactively identifies and communicates operational issues that impact clinical practice.

 

Identify and implement special projects when appropriate to enhance the services offered and ensure continued grant funding

 

Advocates for the profession internally and externally to UW Health.

 

Evaluates and re-evaluates patients/clients using knowledge of the principles and methods of social work assessment and on-going reassessment to provide care to patient and family.

 

Completes accurate and timely department evaluations and documents appropriate decisions and recommendations.

 

Practices in ways that ensure patient care and administrative tasks are performed timely while remaining flexible with changes in schedule.

 

Uses available resources, including patient outcomes and past experiences, to guide patient care.

 

Is accountable and takes responsibility to identify and meet the needs of the patient. Seeks alternate strategies and/or other resources to make sure patient needs are met.

 

Able to provide appropriate care to adult and pediatric patients.

 

Develop and evaluate outcomes of individualized service plans.

 

Assists patients with care navigation, including insurance and benefits eligibility assessment and enrollment.

 

Occasional travel to patient appointments outside of UW Health and to in-state or out-of-state meetings and conferences may be required.

 

Provides treatment interventions to patients that are safe, timely, effective, efficient, patient-centered, and equitable, in compliance with organizational policies and procedures.

 

Demonstrates competency in treatment skills by completing institutional and program-specific competencies and consistently meets annual requirements.

 

Provides effective interventions that are patient centered.

 

Advocates on behalf of patients to appropriate individuals and is aware of common patient needs for similar patient populations.

 

Identifies cultural factors that may impact treatment and plan of care and identifies appropriate resources to meet cultural needs and maximize patient outcomes.

 

Establishes and maintains rapport with patients by recognizing others’ values and adapting interactions to build trust.

 

Consults with or refers to other colleagues/disciplines to meet patient needs.
