JOB SUMMARY

The OTD Aftercare Coordinator provides support, guidance, and direction for the donor family aftercare program of UW Organ and Tissue Donation (UW OTD). The Coordinator serves as a key liaison between donor families and various stakeholders, including hospital staff, partner organizations, and community groups. The OTD Aftercare Coordinator oversees the elements of the program, monitors best practices, and works directly with donor families to provide support and ensure the OTD is meeting their needs. In collaboration with OTD staff and leadership, the OTD Aftercare Coordinator provides input in continuously developing and implementing the program.


ESSENTIAL DUTIES


Reviews and updates all elements of the donor family aftercare program on an annual basis including letters, materials, policies, resources and surveys.

 

Documents all contact with donor families according to policy. Ensures the process for documenting donor family follow up is compliant with applicable policies and procedures and in alignment with national best practices.

 

Documents, supports and coordinates follow up on donor family complaints, in collaboration with DSS Supervisor and OTD Leadership.

 

Provides donor family aftercare which includes providing support, assistance, resources and advocacy to donor families based on their individual needs. This includes but is not limited to phone conversations, letters, emails, and providing resources directly to families.

 

Facilitates the correspondence program which includes confidential correspondence between donor families and recipients in compliance with OTD policies and procedures.

 

Creates and maintains Aftercare Program resources and guidance on their use while applying financial discretion in collaboration with OTD leadership.

 

Gathers donor family feedback, provides observation and feedback, and collaborates with leadership to engage in process improvements projects.

 

Maintains and facilitates the private UW OTD Donor Family Services Facebook group.

 

Serves as an internal resource on grief and bereavement for OTD staff and leadership as needed.

 

Educates and engages with various audiences, such as OTD staff and DSA hospital partners, on the donor family aftercare program and the services offered, the importance of organ donation, and care at the end of life as requested.

 

Participates in evaluations of quality of care and standards being provided by OTD to each donor family during and after a case. This may include case reviews, periodic documentation reviews, survey reviews and observation.

 

Assists with any or all stages of planning, development, and execution of events in collaboration with other UW OTD staff.

Must possess a current, valid driver’s license issued by the individual’s state of residence.
