JOB SUMMARY

The Mobile Integrated Health (MIH) Professional is an integral part of the Home-Based Hospital Care (HBHC) interdisciplinary care team, providing hands-on clinical care and acute support to patients in their home setting. Operating primarily in the patient's home environment, the MIH Professional functions with a high level of autonomy under the direction of the HBHC Medical Director.

 

This role is responsible for conducting clinical assessments, delivering delegated medical care, documenting interventions, and collaborating closely with physicians, nurses, and ancillary staff to ensure the safe, efficient, and patient-centered delivery of acute care at home. In addition to strong clinical skills, the MIH Professional must perform home safety assessments, adapt patient and caregiver education to unique situations, and support care through in-person visits and digital health technologies. Interdisciplinary case conferencing and timely, thorough documentation in the electronic health record are essential responsibilities of this role.


MAJOR RESPONSIBILITIES


Conduct comprehensive in-home assessments of physical, functional, and psychosocial status.

 

Assess the home environment for safety, infection control, and community resource needs.

 

Maintains technical skills according to required standards, as measured by competency assessments during orientation and as required.

 

Implements safe, competent care with HBHC patients and families within policies, procedures, and standards of care.

 

Provide delegated clinical care including vital sign monitoring, wound care, blood draws, IV administration, medication setup, and injections.

 

Identify signs of patient deterioration or risk and escalate appropriately via established protocols.

 

Document assessments, interventions, and patient responses in Epic.

 

Communicate effectively with the broader HBHC care team to support care planning and timely interventions.

 

Support patients and caregivers with education related to medications, symptoms, and home care.

 

Adhere to infection control and safety procedures appropriate to the home environment.

 

Assist with transition planning, community resource linkage, and home safety evaluations.

 

Involves the patient and caregiver in the plan of care, incorporating their cultural, spiritual and other belief systems.

 

Participate in interdisciplinary huddles and ongoing case conferences.

 

Collaborate respectfully with colleagues in nursing, pharmacy, social work, and provider teams.

 

Maintain and manage medical equipment and supplies in field kits.

 

Report equipment, supply, or environmental safety concerns to leadership.

 

Participate in pilot workflows and documentation refinements to support program evolution.

 

Assist leadership in identifying potential clinical practice issues and contribute to the development of specific plans to improve HBHC patients and outcomes.

 

Other duties as assigned.
