• | Coordinates mobility care for inpatient, observation, and short stay patients. Performs all aspects of care in an environment that optimizes patient and environmental safety. | • | Provides education regarding the importance of mobility and fall prevention strategies to both patient and their support systems. | • | Coordinates care with the multidisciplinary care team. | • | Monitors and accurately documents patient response, maintaining confidentiality of information and communicates appropriately to physician or other appropriate member of interdisciplinary team. | • | Provides accurate documentation for patient care provided. | • | Demonstrates critical thinking skills and exercises sound judgement in evaluating and responding to emergent situations according to system practices. | • | Assist, monitors, and prescribes exercise for participants based on established department guidelines. |
