JOB SUMMARY

Provides fall prevention education and mobilization to patients within the hospital setting. 

ESSENTIAL DUTIES

•

Coordinates mobility care for inpatient, observation, and short stay patients. Performs all aspects of care in an environment that optimizes patient and environmental safety.

•

Provides education regarding the importance of mobility and fall prevention strategies to both patient and their support systems.

•

Coordinates care with the multidisciplinary care team.

•

Monitors and accurately documents patient response, maintaining confidentiality of information and communicates appropriately to physician or other appropriate member of interdisciplinary team.

•

Provides accurate documentation for patient care provided.

•

Demonstrates critical thinking skills and exercises sound judgement in evaluating and responding to emergent situations according to system practices.

•

Assist, monitors, and prescribes exercise for participants based on established department guidelines.

As needed, additional responsibilities reasonably within the scope of duties and physical requirements may be assigned.  ALL DUTIES AND REQUIREMENTS MUST BE PERFORMED CONSISTENT WITH THE UW HEALTH PERFORMANCE STANDARDS AND RESPECT FOR PEOPLE COMMITTMENTS.

Clinical

Will provide all patients age and culturally appropriate assessment/screening and interpretation of clinical and laboratory data. Develops and implements age appropriate interventional plans of care including education within the parameters of their position responsibilities and licensure

JOB REQUIREMENTS

Bachelor’s Degree in Exercise Science, Physiology, Kinesiology, Rehabilitation Science, or Health Education with an emphasis in Exercise Science.

Required

BLS Certification.

Required

LICENSE, CERTIFICATIONS, AND REGISTRATIONS MUST BE MAINTAINED PER UW HEALTH POLICY. 

•

Successful completion of annual clinical, age and job specific competencies and skill verification tools.

•

Good interpersonal skills and ability to deal with all levels within the organization and community under varied conditions.

•

Excellent verbal, written, organizational and critical decision-making skills.

•

Strong leadership and adult education skills.
