JOB SUMMARY

The RN – Intake is an experienced registered nurse who is accountable for providing the initial point of contact to patients coming into the UW Health system. The RN - Intake is well-versed in the patient population and understanding the appropriate resources and care teams to help meet the patient's needs. The RN reviews the patient's medical history and referral to determine the most appropriate care team for the patient and then works in collaboration with an Ambulatory Access team member to schedule the patient in a timely manner. The RN - Intake also connects the patient with the appropriate RN Navigator based on the diagnosis or symptom. The RN will demonstrate clinical assessment expertise, triage expertise, and outstanding customer service throughout the interaction for both external and internal users.

The RN – Intake must communicate effectively with faculty, staff, and external contacts using tact, discretion, and diplomacy.  The RN must possess well developed problem solving and team development techniques.  Key responsibilities include direct patient care, participation in staff development and education, and quality improvement initiatives. The RN – Intake delegates and supervises care in accordance with the WI Nurse Practice Act and UW Health Nursing Professional Practice Model. The RN - Intake participates in continuous quality improvement initiatives and assumes responsibility for their professional development.

 


ESSENTIAL DUTIES


Provide initial point of contact for patients coming into the UW Health system.

 

Solicit pertinent clinical information utilizing assessment skills during the intake process to identify and determine the urgency of the requested referral and appropriate care team needs.

 

Serve as a clinical expert to assess and recommend appropriate resources.

 

Utilize triage skills to determine if further testing is required and implements the appropriate ordering process.

 

Assure all necessary medical records are available to assign a navigator.

 

Exhibit sound nursing judgement in decisions involved in the coordinating of multiple, complex case management of patients requiring care and services.

 

Assign and connect patients with the appropriate RN Navigator based on the diagnosis or symptom.

 

Work collaboratively with Ambulatory Access team members to meet the patient’s needs.

 

Facilitate communication among members of the multidisciplinary care team with the initial point of contact.

 

Serve as a role model, demonstrating effective patient care, leadership, and professional behavior.

 

Act as a consultant and clinical resource to staff through provision of patient care and coordination of care.

 

Support patient advocacy, the Patient’s Bill of rights, and UW Health Ambulatory Service Standards.

 

Develop, maintain, and evaluate patient care standards.

 

Maintain clinical competency by attending and participating in divisional meetings and in-service education.

 

Promote open and effective communication among clinical staff, faculty, and referring providers.

 

Responsible for a safe and therapeutic environment for patients, visitors, and staff.

 

Develop and implement efficient systems to enhance patient satisfaction and provider productivity.

 

Interact with other departments, physicians, and staff to assure delivery of quality care to patients.

 

Create an environment in which staff learning is supported and stimulated.

 

Interpret and support the philosophy, objectives, policies, and procedures which guide personnel, patients, and the public.

 

Participate in patient, family, and staff teaching.

 

Develop and update appropriate written patient education materials.

 

Serve as a resource liaison for staff including the regional (outreach) settings to ensure continuity of care between systems.

 

Access and train on various electronic medical record systems which supports the regional (outreach) clinical models to ensure continuity of care between systems.

 

Support and/or participate in research programs approved by the Hospital and University which may be extended to the regional (outreach) clinical settings.
